We had originally planned to use the results of the DAFNE
trial to design a full RCT. However, as we began our planning, it was clear
that the development of such a trial would raise considerable practical
and ethical issues.
In particular we were presented with an ethical dilemma of having to delay
instituting what appeared to be an effective and useful intervention in
our centres while we undertook the trial.
Eventually, after receiving a number of enthusiastic enquiries from other
groups across the UK, we abandoned our plans for a full RCT and decided
in its place to undertake an evaluated roll-out to other centres in England.
This was made possible by a grant from the Department of Health Expert Patient
Programme, with additional financial support from Novo Nordisk.
