Please note that the costs and some of the information in this download are out of date. We are currently updating this resource and the updated version will be available before the end of October 2005. |

| 3. | Once you have secured funding, check the next available course dates for the training of your DAFNE Educators and doctor(s). If your identified Educators and doctors can meet the available course dates, an application form will need to be completed. Each identified Educator and doctor will need to complete sections B and C respectively of the application form. | |||||||||||||
| 4. | 4. Contact Central DAFNE Administration (0191 293 4115) to check if your preferred training programme and dates are still available and provisionally book you training places. In order to secure the training places the completed application form and a non-returnable deposit must be received by Central DAFNE within 10 working days of the date of the provisional booking. Once the completed application form is received by Central DAFNE Administration you will contacted with details of your training plan and additional information. |
