  Additional Information

The following documents contain additional information about DAFNE; we recommend that you read these before you decide whether or not you and your diabetes unit can take advantage of this funding opportunity:

DAFNE Information booklet - gives a summary of what is involved in becoming a DAFNE centre.
Delivering the NSF booklet – further information on how DAFNE can help your service deliver the diabetes NSF.
Key Criteria that structured education programmes should meet to fulfil the NICE requirements.
A list of the training programmes currently available.
A DAFNE application form – to be completed upon the provisional booking of your training dates (see below).

How do you access the funding?

The training grants will be allocated on a first come first served basis.

If you are still interested after reading the information listed above and you can satisfy the above requirements you should:

1. Contact your local Novo Nordisk representative to establish if grants are still available. Your representative should be able to confirm if you have been successful the following working day, if not the same day.
2.

Contact Central DAFNE within 3 working days of being allocated a training grant by your Novo Nordisk representative, to check the remaining training date availability to establish whether these are suitable for your team, and provisionally book (please refer to important note below) your training dates.

   
  If you do not contact Central DAFNE Administration to provisionally book your centre’s training dates within 3 working days of being allocated a grant by your Novo Nordisk representative, Central DAFNE Administration will have to contact Novo Nordisk, to inform them of this and request that the training grant is made available to other units.

Important note

The provisional training dates that you book will only be held for 5 working days. In order to confirm your training dates and training grant, your completed application form and cheque (see point 3 below) must be received by Central DAFNE Administration within 5 working days of making the provisional booking .

Unfortunately, if your centre is unable to accommodate any of the remaining training dates in 2006, your training grant will have to be allocated to another interested unit.

3. Complete the DAFNE Application form and return this along with your £1,000 cheque (made payable to Northumbria Healthcare NHS Trust) to Central DAFNE Administration, within 5 working days of provisionally booking your training dates (see point 2 above).
   
 

The information on the application form is required to ensure the educators and doctor are fully aware of the time commitment required and to provide contact details so that information regarding the training and DAFNE resources can begin to be sent out to the trainees.

   
  Upon receipt of the completed application form and cheque, Central DAFNE will contact:
  Novo Nordisk, to confirm your centre’s training grant
  You, to confirm your training dates and provide further information regarding your training.
     
  If the completed application form and cheque is not received by Central DAFNE within 5 working days of provisionally booking your training dates with Central DAFNE Administration (point 2 above) your training grant will have to be made available to an other centre.
